Botox® for Migraine Prevention

Clinically proven treatment for chronic and frequent migraine attacks

Understanding Migraine and Chronic Migraine

Migraine is one of the most common neurological disorders, affecting an estimated 8–10% of Canadians, or more than 5 million people. About 1–2% live with the more severe form known as chronic migraine — defined as 15 or more headache days per month, with at least 8 having migraine features such as nausea, light sensitivity, or pulsating pain.

When migraine becomes this frequent, traditional oral medications often fail to provide adequate relief, and more preventative therapies like Botox® may be recommended.

How Botox® Helps Prevent Migraine

Botox® (onabotulinumtoxinA) is a Health Canada–approved preventive treatment specifically for chronic migraine in adults. It works by blocking the release of pain-transmitting chemicals and calming overactive nerve endings, which helps reduce the number and intensity of migraine attacks over time.

Clinical studies show that patients receiving Botox® experienced:

  • An average reduction of 8–9 headache days per month
  • Fewer migraine attacks and medication days
  • Noticeable improvements in daily functioning and overall quality of life

While not all patients respond the same, many notice gradual improvements after their second or third treatment cycle.

Is Botox® Right for You?

You may be a candidate for Botox® for chronic migraine if you:

  • Experience 15 or more headache days per month for at least 3 months
  • Have tried other preventive medications without sufficient relief
  • Are seeking a non-narcotic, minimally invasive treatment

Your migraine specialist doctor or nurse practitioner will determine eligibility and create an individualized treatment plan based on your symptoms, medical history, and treatment goals.

What to Expect at the Headache, Migraine & Concussion Centre (HMCC)

At HMCC, we use the PREEMPT protocol, the same evidence-based standard used by leading academic headache centres worldwide.

During your appointment:

  • The procedure takes about 15–20 minutes
  • Botox® is injected into 31 small sites across 7 areas of the head, neck, and shoulders
  • Treatments are repeated every 12 weeks
  • Full benefits are typically achieved after 2–3 sessions (6–9 months)

Our medical team ensures your comfort, safety, and personalized care at every step.

Cost, Coverage, and How to Access Botox Treatment

Botox® for chronic migraine is an approved treatment covered under the Ontario Drug Benefit (ODB) program for eligible patients who meet specific medical criteria. This means that your consultation appointment at the Headache, Migraine & Concussion Centre (HMCC) is covered by the Ontario Ministry of Health.

The medication itself (the Botox® drug) may also be covered under the Exceptional Access Program (EAP) of the Ontario Ministry of Health. This program provides access for patients who:

  • Have been diagnosed with chronic migraine (15 or more headache days per month, lasting 4 hours or longer each day)
  • Have tried at least two other preventive migraine medications without sufficient relief or tolerance.

Our heath administrative team assists every step of the way, including:

  • Completing and submitting your EAP drug-coverage application to the Ministry of Health
  • Coordinating with your pharmacy and insurance provider for coverage confirmation
  • Guiding you through private or workplace plan reimbursement if you are not eligible for public funding

We understand that navigating coverage can be stressful, and we strive to make the process as smooth and transparent as possible so you can focus on feeling better.

Our multidisciplinary model allows patients to receive integrative, continuous care under one roof, ensuring both preventive and acute management options are available when needed.

Follow-Up & Ongoing Evaluation

Improvement is typically assessed at 12- and 24-week intervals. Our care team tracks headache frequency, medication use, and quality-of-life scores to evaluate your response and adjust treatment if needed. For partial responders, combination therapy (such as Botox® with CGRP-targeted medication) may be discussed.

Safety & Side Effects

Botox® is generally well tolerated when administered by a trained clinician.
Common side effects: mild neck soreness or temporary injection-site pain.
Less common effects: muscle weakness, drooping eyelids, or localized discomfort — typically short-lived and self-resolving.

Serious side effects are rare but can include difficulty swallowing or breathing; patients are advised to seek immediate medical attention if these occur.

Before treatment, your provider will review your full medical history, medications, allergies, and previous responses to migraine therapy to ensure that Botox® is safe and appropriate for you.

Botox® should only be administered by trained medical professionals.
Serious side effects are rare but can include difficulty swallowing or breathing if the toxin spreads beyond the injection site. Always inform your provider of any neuromuscular conditions or medications you are taking before treatment.
